Mandated BMI Screening for 10th Graders

Old Colony will be conducting the State mandated BMI Screening for 10th graders during the first week of June.   The purpose of this screening is to identify signs of possible weight related issues. A simple height and weight will be taken and recorded.  Complete privacy will be provided.   It is not a diagnostic service but a program to find young people who may need further evaluation.

If a child has unusual findings, the parent/guardian will be notified and asked to take their child to a physician as a precaution.

If a parent/guardian does not wish to have their son/daughter participate,they must download this form, sign it and return it to the  school nurse before June 1, 2013.
